Thank you for purchasing Equiflavin from Equine Health Solutions (EHS). To activate your $10,000 colic surgery warranty, complete the appropriate online registration form below within 15 days of purchase. This warranty is backed by Hartwell Insurance in Nashville, Tennessee.
Please note:
Your horse must not have had colic surgery in the previous 365 days to qualify for standard enrollment.
You must administer two 5ml doses of Equiflavin weekly.
In the event of colic, administer at least 10ml of Equiflavin immediately.
Annual usage requires approximately 520ml per horse.
If your horse has had colic surgery within the past year, use the Exception Review Form.
